I believe that form is 8610, which is signed by the FSDO after they have reviewed your documentation of 2500 hrs *each* of supervised maintenance on engines and airframes. A certificate from an approved school also works. Then, you are allowed to take the oral, written and practical tests for an AMT, airframe and powerplant.
In many FSDOs, I'm told that they no longer count homebuild time as it is not "maintenance". Anyone know more about what is current practice?

That's correct, the form is an 8610-2. This must be filled out in duplicate. The recommendation letter, typically written by the A&P who is making the recommendation to the FSDO, should include a statement, ".........meets the requirements of FAR 65.77(b) for airframe and powerplant work experience."
