I know this must be a dumb question, but is there a foolproof safety device to prevent starting the left engine with the crew door ajar and to prevent opening the door with the engine turning?

Joel,
The Turbo Commanders have an electric lock that must be over-ridden if the engine is running.
The piston models depend on common sense. If you don't have a supply of that, you can subscribe to a Safety Management System, become IS-BAO 9000 certified and have a big fat manual with a specific procedure written to prevent yourself from opening the crew door with the engine running.
Either method will work.

In the interest of accuracy, the electric door lock is active when the Master Switch is on on the Turbo Commanders and the AC685. I know Sir Barry is watching ...

Joel,
To the best of my knowledge such a system does not exist. My plane has a plackard by the door window which advises that only the pilot is allowed to open, shut, or lock the door.

As we know, all too well, various “advanced” NAAs such as FAA and our CASA mandate the second option.

Not only the turbine commanders. My 685 had the same system.
The unlocking switch is the far right one on the overhead panel
