I've taken apart the primer handle to replace the o-ring and after the reassembly, it will go into cylinder or system position but it cannot be centered. Has anyone encountered this before? I've disassembled and reassembled it a few times now.

Yes. If I remember correctly, I had to Rotate the plunger 180 degrees and reinstall the plunger.
