Probe. If you have a connector in line with the probe you might get lucky and find that is the problem - for that matter look over the probe wire itself to see if it has chaffed or otherwise damaged.

Good question, simple answer:
You have a loose and/or corroded connection.
You cannot inspect visually.. Remove the connections, clean them, replace the star lock washers with new. WHAT'S that you say... No, star lock washers? That's the other part of the problem.
