Scott or Craig---- I am getting ready to order the actuator 5-1577-4 and controller 5-1577-C as recommended below.

 Question: Should I order 3 each of the: 11-3064 momentary switches for later installation or is controller all that  I'll need for operating the unit.

The controller is all you need. You can use external  buttons if you can't fit (or for some reason don't like) the buttons on the  controller. The spec for the controller is at www.spal-usa.com/actuators/automated/tech_sheets/LA-CONTROLLER.pdf
