It looks like the NTSB finished up their examination of Ken Lien's fatal
crash from back in August of 2008.

The long and the short of it is this:

.onsite examination of the airplane revealed that the carburetor's
mixture control lever was observed separated from its mixture shaft.

The full narrative:
http://www.ntsb.gov/ntsb/brief2.asp?ev_id 080923X01519&ntsbno=SEA08LA190&akey=1

I hope all builders including myself can learn something from his tragedy.

I noticed another comment - the final sentence of the NTSB report:
The mixture control cable was secured to the firewall, not to a
bracket on the engine.

This suggests to me the installation of the engine was not done
correctly. The report also indicates this was the maiden flight of the plane.
