There are no enforcable rules as such. As someone pointed out, AC43-13 has the
guidance for washer usage. This advisory circular (technically, not a rule)
applies to type certificated aircraft.
If your aircraft is licensed as an experimental, then the methods described
there are usually wise to use on your aircraft, but not mandatory. They are
simply good practice. For whatever it's worth, I pretty much follow AC43-13
on my experimental.
I have never seen anything in a FAR address washer usage, but that's not to
say it may not exist somewhere.

What are the rules regarding usage of flat washers on the AN bolts? I assume
you must have one under the nut so you don't scuff the metal it is
tightening against (when you tighten it)? How about the other end, is it
necessary or even required by FARs?
How about on the rudder pedal where you have a bolt going through a clevis
that attaches the cable? Is a washer under the castle nut enough or do you
need one under the head as well?
