I got an inexpensive one at Fry's Electronics in Calif. - There, you can also get a nice durable and inexpensive grill to cover the speaker.
There is a specific OHM requirement which is probably in the manual for the audio panel. I cut a hole in the side panel at hip level in the right side of the back seat. Seems to work fine, but with all the excellent headsets out there - not sure it is really needed.

Not required. Just another thing to break. Put jacks at all four seats and
an aux jack on the panel.

I mounted a 11-01569 from ACS in the upper section of the rear bulkhead.  Works great.

We put a doubler to beef up the cut in the corrugation, then the interior cloth went right over the speaker so it's invisible.

It's about 2 feet fwd of the AFS magnetometer, but it doesn't seem to affect it.
